Peptide versus small molecule

Semaglutide is a chain of amino acids, and the gut exists to break those down. Rybelsus gets around that by pairing each tablet with 300 mg of SNAC, salcaprozate sodium, which raises pH locally in the stomach so a sliver of the dose crosses the lining before it is destroyed. The label puts absolute bioavailability at 0.4% to 1%. Food, a large glass of water, or another pill taken at the same time all cut that further, which is where the fasting rules come from.

Orforglipron is not a peptide. It is a conventional drug molecule, about 900 daltons, that activates the same GLP-1 receptor. It survives digestion on its own. The Foundayo label reports 77% bioavailability, a peak at 4 to 8 hours, and steady state after about a week. With food, exposure fell 19% in the label’s study and about 18% to 24% in the published food-effect trials, which the label calls not clinically relevant. So the instruction is one line: once daily, with or without food, swallow whole.

The head-to-head

ACHIEVE-3 randomised 1,698 adults with type 2 diabetes inadequately controlled on metformin to orforglipron 12 or 36 mg or oral semaglutide 7 or 14 mg for 52 weeks. It was open-label. Orforglipron won on the primary endpoint at every pairing, including its lower dose against semaglutide’s higher one.

Weight followed the same pattern: 9.2% on orforglipron 36 mg and 6.7% on 12 mg, against 5.3% on semaglutide 14 mg and 3.7% on 7 mg.

The cost showed up in the safety table. Gastrointestinal adverse events hit 58% to 59% of the orforglipron groups versus 37% to 45% on semaglutide. Discontinuation for adverse events was 9% to 10% versus 4% to 5%. Pulse rose by 3.7 to 4.7 beats per minute on orforglipron versus 1.0 to 1.5 on semaglutide. More drug reaching the bloodstream means more effect and more side effect, in the same trial.

One caveat that matters. ACHIEVE-3 is a diabetes trial, and Foundayo is not approved for diabetes. It is the cleanest evidence that the two molecules differ, but it is not evidence for either product’s approved use against the other.

Foundayo is approved for weight reduction in adults with obesity, or overweight with a weight-related condition. The evidence is ATTAIN-1, 3,127 adults without diabetes over 72 weeks, and ATTAIN-2, 1,613 adults with type 2 diabetes.

In ATTAIN-1, 54.6% of the 36 mg group lost at least 10% of body weight and 18.4% lost at least 20%, against 12.9% and 2.8% on placebo. In ATTAIN-2, the same dose produced 9.6% weight loss versus 2.5%, the familiar blunting that diabetes causes with every GLP-1. Discontinuation for adverse events ran 5% to 10% across both trials.

Rybelsus is approved for type 2 diabetes. Its evidence is the PIONEER program. In PIONEER 4, 14 mg lowered HbA1c by 1.2 points over 26 weeks, matching injected liraglutide, with 4.4 kg of weight loss. Higher tablets exist: in PIONEER PLUS, 25 and 50 mg beat 14 mg on HbA1c, and the 25 mg tablet reached the market as Wegovy, not Rybelsus, on the strength of OASIS 4. Rybelsus itself has no weight-loss indication and no cardiovascular indication.

Orforglipron’s diabetes data are just as extensive. ACHIEVE-1 tested it as monotherapy in 559 adults with early type 2 diabetes: HbA1c fell 1.24 to 1.48 points over 40 weeks against 0.41 on placebo, with 7.6% weight loss at the top dose. That indication is not on the label yet.

One tablet, two sets of numbers

The trial papers describe orforglipron as 3, 6, 12, and 36 mg. The box says 0.8, 2.5, 5.5, 9, 14.5, and 17.2 mg. Lilly’s later analyses report the ATTAIN arms as 5.5, 9, and 17.2 mg, so the marketed strengths are the trial arms under a new naming convention, not different doses. When this page cites 36 mg, it is describing the 17.2 mg tablet.

Both ladders move on a 30-day cadence, unusual for daily drugs. Neither is limited by how fast the drug accumulates; orforglipron reaches steady state in about a week. The interval is about letting the gut adapt.
