Why 14 mg by mouth is not 14 mg by injection

Semaglutide is a peptide. Swallowed on its own, stomach acid and enzymes destroy it. The tablet pairs each dose with 300 mg of SNAC, salcaprozate sodium, which raises pH in its immediate vicinity and lets semaglutide cross the stomach lining before it is broken down.

Even so, only 0.4% to 1% of a Rybelsus dose reaches circulation, per the label’s pharmacokinetics section; the newer Ozempic tablets manage 1% to 2%. That is the arithmetic behind the ladders: a 14 mg tablet and a 1 mg injection are two different ways to get a similar amount of drug into the blood. What the trials showed

The two products were never compared directly. Ozempic came from the SUSTAIN program; Rybelsus from PIONEER. The best single PIONEER trial for this page is PIONEER 4, because it put the tablet against an injected GLP-1.

At 26 weeks, oral semaglutide 14 mg was non-inferior to liraglutide 1.8 mg on HbA1c and produced more weight loss: 4.4 kg versus 3.1 kg, against 0.5 kg on placebo. In PIONEER 1, as monotherapy over 26 weeks, placebo-adjusted HbA1c reductions were 0.6, 0.9, and 1.1 points for 3, 7, and 14 mg, and only the 14 mg dose produced meaningful weight loss, 2.3 kg.

The tablet also has headroom that the approved doses do not use. In PIONEER PLUS, 25 and 50 mg tablets beat 14 mg over 52 weeks.

Those higher doses are not approved for diabetes. The 25 mg tablet did reach a label, but as Wegovy for weight reduction, where it produced 13.6% mean weight loss over 64 weeks in OASIS 4.

For the injection, SUSTAIN FORTE is the relevant dose comparison: 2 mg beat 1 mg on HbA1c over 40 weeks with about 0.9 kg extra weight loss. And SUSTAIN-6 gave Ozempic a cardiovascular indication, with major events at 6.6% versus 8.9% on placebo. Rybelsus has no such indication.

Both starting steps exist for tolerability, not effect. The 3 mg tablet is not a treatment dose; the label uses it to let the gut adapt before 7 mg. The oral semaglutide label describes moving to the injection at its own 0.5 mg step, and moving from 0.5 mg injection to 7 or 14 mg tablets. It does not convert milligrams across routes.

The empty-stomach rule

The Rybelsus label is unusually specific, because absorption is fragile:

  1. On waking, before anything else.
  2. With no more than 4 ounces of plain water. Not coffee, not juice, not a full glass.
  3. Swallowed whole. Not split, crushed, or chewed.
  4. At least 30 minutes before food, other drinks, or other oral medicines.

Each rule maps to a measured loss of absorption in the pharmacology studies. Ozempic has no food rule at all.

The brand map has moved

Semaglutide brands used to map onto routes: Ozempic and Wegovy injected, Rybelsus swallowed. Two changes broke that.

Ozempic-branded tablets in 1.5, 4, and 9 mg now sit on the same oral semaglutide label as Rybelsus. And the Wegovy label carries a tablet in 1.5, 4, 9, and 25 mg for weight reduction.   • Is Rybelsus as effective as Ozempic?

    They were not compared head-to-head. In its own trials, 14 mg oral semaglutide lowered HbA1c by about 1.1 to 1.2 percentage points and matched injected liraglutide. Ozempic 1 mg lowered HbA1c by about 1.9 points in SUSTAIN FORTE, and 2 mg by more. The labels treat the two as separate products with separate ladders.

  • Why is the Rybelsus dose so much higher in milligrams?

    Absorption. Semaglutide is a peptide and is destroyed in the gut. The tablet pairs it with SNAC, which raises stomach pH locally and lets a small fraction cross the stomach lining. The label's pharmacokinetics section puts absolute bioavailability at 0.4% to 1% for Rybelsus, so 14 mg by mouth delivers something in the range of a 1 mg injection.

  • Can I take Rybelsus with food or coffee?
